Insurance Waiver Forms

Let's get the party started. Anyone under 18 needs to ask their Mum or Dad to sign a Insurance Waiver prior to play. Gamers over 18 can sign a form for themselves.

Complete our online waiver form, or bring a completed printed form with you on the day to hand to our registration staff.

Before You Battle – Download Your Waiver

All participants need to complete an insurance waiver aka 'Membership Form'.
