Membership Forms

Let's get the party started. Anyone under 18 needs to ask their Mum or Dad to sign a Membership form prior to play. Gamers over 18 can sign a form for themselves.

Please note we do NOT accept digital copies. You must bring the printed form with you on the day and hand it to our registration staff.

All participants need to complete a Membership Form.
